MP Ration Card Apply: Eligibility Slip Application

Before you apply: check two things

1. Does your family have a Samagra Family ID? The form asks for it, and for each member's Samagra ID. If you do not have one, register with Samagra first through your Gram Panchayat or urban body office.
2. Do you fit an eligible category? Only Antyodaya and priority households under the state's notified categories qualify. News reports say MP has 28 categories. Read MP ration card eligibility before applying.

Step by step: offline application

1. Get the official form "राशन पात्रता पर्ची हेतु आवेदन" from your Gram Panchayat (rural) or urban local body / ward office (urban).
2. Fill in the head of family's name and Samagra Family ID, father's or husband's name, age, gender, mobile number, Aadhaar number and address.
3. List every member with their Samagra ID.
4. Tick or write your eligibility category.
5. Read the declaration carefully (see below) and sign.
6. Attach: the Samagra Family ID, proof of your eligibility category, and Aadhaar of all members.
7. Submit the form at the Gram Panchayat or ward office and ask for a receipt or acknowledgement.
8. Wait for verification. When approved, you can check the slip on the Ration Mitra patrata parchi page. See khadyan parchi MP.

Office procedures vary. If the staff ask for something different, follow their instructions and keep copies of everything you submit.

The declaration: who cannot apply

The official form's declaration states that:

• no family member pays income tax, and
• no member is a Class I, II or III employee of government, an autonomous body, a nationalised bank or a cooperative.

A false declaration can lead to cancellation. Nationally, the food ministry flagged over 1.17 crore possibly ineligible cardholders in August 2025, including income-tax payers and four-wheeler owners, as reported in news. Apply only if you truly qualify.

Can I apply online for an MP ration card?

A secondary source describes an online route: apply on mpedistrict.gov.in or at a Lok Seva Kendra for the service "Application for BPL Ration Card / Food Security Eligibility Slip", followed by field verification by the Janpad CEO (rural) or the CMO (urban). This is not confirmed from any official source we could read, so we do not link to it. Ask at your Lok Seva Kendra or Gram Panchayat whether the online service is available in your area. The paper form to the Gram Panchayat or ward office is the route shown on the official form.

After you apply

• Verification: as reported by ETV Bharat in February 2026, new applicants are checked for Aadhaar, bank account, Samagra ID and mobile. Keep your Samagra, Aadhaar, bank and mobile details consistent so verification does not fail.
• Waitlist: under the TPDS (Control) Amendment Order 2025, as reported, new cards are to be issued first-in-first-out with a real-time waitlist on state portals.
• e-KYC: once approved, every member should complete e-KYC at the ration shop. See MP e-KYC.
• Check status: look up your district on the MP ration card list or your slip on Ration Mitra.

Delays and complaints

If your application is pending for long, ask at the office where you submitted it. Then complain on the CM Helpline 181 (MP Samadhan). The food helpline is 1967, and the department's toll-free number on scm.mp.gov.in is 1800-3456-194. See our helpline guide.
